If you're a middle class traveler who will be staying in a decent hotel, getting about in a reliable vehicle, and not doing anything stupid, local crime won't be much of an issue.

Crime creates serious quality of life problems that will be more obvious if you want to "experience the city." That's still not necessarily a threat to your safety, but it's where situational awareness becomes important. It also means making compromises. Maybe you're not comfortable letting your teenager have a day on their own, maybe it's not smart to walk from the restaurant to the hotel after dinner with your wife, maybe you need to call company security to escort you around the new construction site, maybe you need to drive an extra few miles for a bathroom break, and so on. I don't like making compromises.

Yes, there are plenty of small towns in the US where you need to be careful too.

None of the above, although some I would likely not find very interesting. The only cities worldwide I wouldn’t visit over crime are a few in Central and South America and even then the list is fairly minimal because I have a fairly high danger acceptance threshold.

If you're afraid of visiting any of these places it'd probably be easier to compile a list of things you aren't afraid of

If I lived in Canada, all of them. Guns don't respect state lines, sadly.

Probably only Gary and East St Louis, less because of the crime but because there's exactly Jack all to do there to warrant any crime risk, or even just the gas money.

Even then, say if I were visiting friends in St Louis and they suggested we go out to the couple casinos in East St Louis for the evening, I'd say sure.

This is basically me as well.
