Not quite the answer you are looking for, but this post looks at Mondale counties which flipped to McCain, and Dole counties which flipped to Obama:

For some reason, the Atlas is misbehaving for me tonight. But I have so far uncovered Trinity and Nevada Counties in California. Trinity voted for Perot in 1992 but hadn't voted Democrat since 1964; Nevada last voted Democrat in 1964.

Also, Carson City and Washoe County, Nevada. Arapahoe County, Colorado. Los Alamos County, New Mexico. Lancaster and Douglas Counties, Nebraska. Brookings County, South Dakota. Grand Forks and Cass Counties, North Dakota. Olmsted County, Minnesota.

Teton County, Idaho last voted Democratic some time before 1964. Same is true of Gallatin County, Montana.

That's everything west of the Mississippi so far.

Teton County voted for Truman in 1948. Gallatin voted for Roosevelt in 1944.

As has been mentioned before, Boone, Carroll, DuPage, Kane, Kendall, and McHenry Counties in Illinois voted for a Dem for the first time ever this year (the only time they haven't voted for a Republican since the Civil War was 1912, when Roosevelt won them all but Carroll).

Some others:
Jefferson County, AL in '52 (voted for Wallace in '68)
Arapahoe County, CO in '64
Jefferson County, CO in '64
Ouray County, CO in '64
Stephenson County, IL in '64
Berrien County, MI in '64
Jackson County, MI in '64
Kent County, MI in '64
Leelanau County, MI in '64
Douglas County, NE in '64
Lancaster County, NE in '64
Los Alamos County, NM in '64
Cass County, ND in '64
Grand Forks County, ND in '64
