Optiscan forms are used in many precincts in IL, in many precincts like mine you can vote with either an electronic machine or by optiscan. Optiscan was the preference in my precinct, and apparently in Chicago as well.

Here are the basics of optiscan voting. You get a ballot that look like a large scantron form and black felt-tip pen to use with the ballot. The ballot is marked with candidate names and propositions and ovals next to each choice. To vote an oval is filled in with the felt-tip pen. After the ballot is complete, the ballot is read by machine and the vote is recorded. If the ballot has errors such as two votes in the same race then it is returned to the voter who can choose to get a fresh ballot and revote.
