So what happens if instead of deciding to direcly colonize Africa the euros opt to do a pattern of proxy wars/client states?

Well that is what you had up until the late 19th century.   However, the 19th century advances in medicine combined with the politcial and economic situation in Europe of the time mean that I can't see anything other than the scramble happening.  To prevent the scramble, you'll need some other change to occur that would make the scramble unattractive to the European powers.  No development of quinine is one possibility, as it would make colonization far less attractive.
