My two cents:

If it’s a small regional cluster, with no established history of symptoms in the region, then it’s unlikely to be genetic. It has to be a pathogen, or an environmental contaminant.

If it was bacterial or viral, the biopsy or lumbar puncture would probably show a high titre of a well-known strain or family.

So, either it’s something prion-related (but novel enough to evade detection thus far), an environmental toxin (some neutotoxic chemical that’s been overlooked), or something entirely novel.

Definitely interesting.
