The full saga is that Cantwell asked for UC for a collection of military promotions outside of the scope of this thread, plus Wormuth's confirmation. It was agreed to.
Then over two hours later (the senate was in session late for amendment votes on an unrelated bill), Schumer asked UC to reverse only Wormuth's confirmation, and the reversal was permitted.

I am not sure if this was as simple as someone accidentally putting "135" instead of "136" on Cantwell's script (Wormuth was right next to all the military promotions on the calendar), or if something on the merits of the nomination came up last minute....

When does a confirmation become official such that the Senate cannot just reverse it?

Politico, c. tomorrow: "Why the Vitiation of Christine Wormuth's Confirmation Proves #DemsInDisarray!"

When does a confirmation become official such that the Senate cannot just reverse it?

I'd presume once the nominee is formally appointed to a given office by the President after being advised-&-consented to.

I'd also presume that this is the sort of thing that can only be done by unanimous consent, since the motion to reconsider was laid on the table by Cantwell's order, which naturally requires bipartisan agreement. Otherwise we'd see McConnell forcing tons of motions to proceed to business of this type as part of his conquest to try to divide Democrats. (any senator can make a non-debatable motion to proceed for any executive calendar business)

I just hope this was some stupid staffer putting the wrong number on Cantwell's script, and not something big....

Soldier’s missing pay triggered Army secretary double confirmation snafu

As I suspected. Senator Cramer (R-ND) was unhappy that one of his constituents, deployed overseas to Africa, was not being paid properly.

I understand how he can hold up a nomination all by himself but not reverse it. Still very unclear on that and why there was unanimous consent for the reversal.

Schumer agreed to the reversal as an accommodation to the Senator, since his issue was one that was resolvable. These are the sort of bipartisan comity things that Atlas simply doesn't understand.

Yeah that's fine. But why not give him what he wants instead of revoke the nomination so he can use it as leverage to get what he wants, and then acquiesce.

This may sound naïf, but how on earth is a well-run, competent government not fully staffed 5 months into a 45-month presidency (11%)?

Because there are hundreds of different executive positions that the Senate can confirm people to.

I still think it's a good point though. We could change the law to allow a lot of positions to be directly appointed by their superiors in the org chart, without need for a Senate confirmation. And even if we don't do that, there is nothing stopping the Senate, other than its own rules, from confirming dozens of nominees at once in a single vote.

There is no way to set that up procedurally except:

1) Unanimous Consent (naturally requires support of every senator)
2) Some sort of mini-nuclear option procedure, which creates precedent that is difficult to dispose of, and which Manchin won't acquiesce to anyways.

I agree that dysfunction is there but the point is that it's ridiculous that we can't fill these spots much more quickly and is unbecoming of us that we don't have a better system in place.

Do ambassadors also need to be confirmed by the US senate?

I've just learned that the person nominated to be US ambassador to my country (Cameroon), Christopher Lamora, is gay. He made a video where he mentioned his husband Eric and a lot of people are loosing their sh*t here. Being gay myself (but hidden, obviously), I'm secretly hoping he gets confirmed, he'd be the first and only openly gay official (since same sex relationships are illegal and harshly punished here). His impact can only be positive for the community.
