Actually the pope is not a monarch as the pope only heads the Catholic church. The Vatican is officially ruled by a king that just so happens to always be the same person as the pope.

President of the Council of State, it sounds familiar to the title of French Prime Ministers until 1958, which was President of the Council of Ministers (Président du Conseil des ministres).

I think the official designation is a "Sovereign", not a "King". It was during the existence of the Papal States the Pope was frequently refeered to as the "Pope-King". A vast majority of sources list the Sovereign of the Vatican as a ecclesiastical, elected monarch. So yes, the Pope is not even a head of state, but a religious leader.

The Captains Regent of San Marino, without question. Two of them who co-lead at all times, and elections are every six months. Also one of them personally greeted me when I visited San Marino. Also San Marino is the country with most female heads of state. Cool name, cool system all-around.
