:/ Not really an ideal election. I guess I would reluctantly pick the Democratic Party as the least bad of the major options. I can respect some aspects of M5S, but they are a bit too extreme on Euroscepticism for me. With no real liberal democrat party active in Italy these days, my first choice would probably be Popular Alternative. But its so tough to know what a party is about when they only last for a few years.

They will form alliances in the actual election. The conservatives will form a pole centred around Forza Italia (although the PM candidate will not be Berlusconi, who is barred from the role due to his conviction; it's likely to be Antonio Tajani iirc). Although the Northern League have overtaken FI in the polls, FI is a more likely "main" coalition partner because of legacy reasons, and also because Brothers of Italy hate the Northern League (although both are far-right, the former stress Italy as a strong unifued state, while the League even with Salvini's makeover has autonomist and separatist elements.

The left will also form a pole around DP, although who knows? Maybe there will be a second one (although both previous elections have had  hard Left coalitions running separate from DP, neither of which hit the threshold).  Fwiw MDP is an association of dinosaurs like D'Alema and Bersani, SI is a continuation of Vendola's SEL that have merged with some DP dissidents and some ragrag left groups (I believe it has also swallowed Pepe Civati's little faction and Progressive Camp is an outfit led by the former Mayor of Milan Giuliano Pisapia. All of them will probably be in alliance with DP, although SI might try and run on their own. (Same with the Greens, who spent the last two elections fruitlessly in he aforementioned Hard Left coalition)

Don't know what will happen with Alfano's little group though.

Yes, I am disappointed. Maybe I’m naive about it, but I would trust M5S’s leadership over Trump any day since many of them seem to at least be able to do the job

Because Renzi's way of leading his party tends towards the "steamroller" style, which has irritated some of the lefties and (more significantly) frayed the egos of bitter old men.
