In this scenario, there is no 22nd Amendment and President Ronald Reagan runs for a 3rd Term. The Democrats nominate rising star Senator Al Gore.

Who is Gore's VP? I am assuming Reagan keeps Bush on the ticket.

I will be honest I think he wins all 50 mainly because of Berlin Wall and ussr crashing at the same time in the middle of the election.

I dont think so , the Democratic party was in far better shape in 1988 than in 1984(when they nominated the vp of a very unpopular president ), and there will be some party fatigue.  Also with Gore dems do much better in upper south and working class areas allowing them to reduce the GOP below 360 electoral votes(and against Bush its under 300)

Largely agree with your prediction. If I were to make any changes I would give Reagan your home state of Oregon as it was still a swing state and Reagan is from the West Coast.

Gore does perform better than Carter and Mondale before, but still falls short.

I don't think he would have won any states in South. The conservative Reagan was a good fit for the region. Only AR may have been close.

✓ President Ronald Reagan (R-CA)/Vice President George Bush (R-TX): 350 EVs.; 52.0%
Senator Al Gore (D-TN)/Former Governor Reubin Askew (D-FL): 188 EVs.; 47.1%

Gore would severely under perform Dukakis at pretty much anything at that point.

Gore would severly under-perform Dukakis outside the South but would do much better in the South, winning (at least) Tennessee, West Virginia, and Arkansas.
