Any city with a population of over 200,000 people was included provided that no state had more than five cities. I did include Grand Rapids even though the 2015 estimate was slightly below 200,000 because there is a decent chance that it is has hit 200,000 by now.

I think that's the jist of it.  I've never heard anyone seriously be like "Yeah, I have to pick up my friend Bob in the Minnie Apple"... they'd say "I have to pick up my friend bob in the Cities"
