While there is some truth to the zeroes thing, there was never a time when a rogue person (or even small group) could by him/themselves launch a nuke from a Minutemen silo.  There were (and are) several layers of security.  The leaving the nukes on a B52 then flying it from Minot ND to Barksdale LA without telling, you know, the pilots or Barksdale!.....a couple of years ago was a bigger breach of security.

I think that was because noted crazy person Curtis LeMay was at the peak of his influence, so SAC had loads of airborne alert planes that made the risk of accidents higher.

I walk past a lovely plaque for the man and through a building named after him every work day.  We're moving buildings in a couple of years and I'm sure the new one will not be named after him.
