We are in bizarro world.  Presidential elections still happen every four years, but the cycle is moved up two years starting in 2002.

A year after 9/11, President Bush is planning to invade Iraq.  Democrats have a difficult time deciding whether to support or oppose Bush's war plans.  IRL, Republicans fared better in the 2002 elections than Democrats.

Would they have been as successful with Bush at the top of the ballot?  Would John Kerry have still been the Democratic nominee, or would Howard Dean have had the issue he needed to win with the war so front and center?

Bush would have steam rolled over anyone in 2002. personally i dont think either Kerry or Dean would have gotten the nomination, i believe that Clark would have been a much stronger candidate, but would still get creamed in the election.
