I've been called "Doctor Rectangle" about twenty times--mostly ironically.  I've known very few who insist on the title and none who use it in groups that include other Ph.D.s.

I've meet a few women who object to the term "Mrs."  All were married.  The general argument is that "Mrs." implies ownership by a husband.  I don't know were they get this from.
