Only 5 with absolute certainty (note that the wording does not require that evolution be true, only that evolution be reasonable; I would give evolution's truth the same qualification I give the non-existence of God, that there is an obscurely remote chance that it isn't the case).

Of them, evolution is the only one that I think is "extremely likely"; and prayer curing medical conditions, I find likely, but only because of psychological reasons.  I also tend to lean toward God not existing, although that's perhaps mostly because I am ultraskeptical of a god in the Christian sense of the term.

Again, the wording comes back to bite you with the prayer question. Prayer heals some problems due to the placebo effect, but it does so indirectly, not directly. (Incidentally, a Christian would also not believe that prayer heals medical issues directly as God must heed the prayer and do the healing himself.)
