Suppose John McCain remained healthy enough to continue serving as US Senator into 2022 or possibly beyond. What do you think would have happened to him in 2022? (Note that option 2 refers to losing a primary contest, whereas option 3 refers to losing a general election contest.)

He retires 100%. If he runs he loses the nomination as bad as Liz Cheney did.

Definitely retires.

And if not, he loses renomination by a margin of 10-15 pts. in the primary. Richard Lugar 2.0.
