I'd guess that state/province boundaries would also be different somewhat.

1861-1900

The history of the United States remains the same until after the Civil War.  Due to a freak accident, General Grant is killed during the war. The Union continues to be lead by a series of inept generals. Lee wins the Battle of Gettysburg and the Army of Northern Virginia continues north towards Philadelphia and New York. In an effort to prevent the Confederates from sacking New York, the city secedes from the Union and declares itself the Free City of New York. It retains close ties with the United  States, however. Confederates occupy Washington, DC.

Per terms of the Treaty of Richmond, Maryland, Missouri and Kentucky secede and joined the Confederacy. Virginia refuses to accept the loss of the western counties and they remain part of that state. Unable to have its capital surrounded by a foreign nation, the United States is forced to relinquish Washington.  The Confederate capital is moved from Richmond to Washington. Bowing to pressure from what is left of its western frontier, the United States establishes the Federal District of Chicago as its new capital.

Separatist movements culminate in Texas declaring its independence from the Confederacy in 1869 and re-establishing the Republic of Texas. Less than a year later, Texas annexes the Oklahoma Territory. On the heels of gold rush fever, California secedes from the United States in 1868 and immediately annexes the new state of Oregon and Washington Territory. The United States is powerless to prevent it. Separtist Mormon settlers in Utah territory join California and establish the semi-autonomous district of Deseret.

The Guilded Age brings prosperity to all four American nations, the US in particular. The Confederacy, which lacked much industrial base after the War for Independence, invests heavily to catch up. Kansas City, Birmingham and Atlanta become major industrial hubs. By 1892, the Confederate Constitution is amended to outlaw slavery, but fails to grant sufferage.  The Confederacy becomes a naval power.

In 1898, the Confederate battleship Georgia sank in Havana, Cuba. Confederate President Philip McKinney blames Spain and uses the incident to further Confederate designs in the Caribbean. “Remember the Georgia!” is heard throughout the south. On April 20, 1898, the Confederate Congress declares war on Spain.

Fearing unfettered Confederate domination of the Caribbean, US President William McKinley orders US forces to occupy Puerto Rico. A standoff ensues with the CSA. British support of the US action prevents further hostilities between US and CS forces.

The War ends on December 10, 1898. Spain cedes Cuba, Guam and the Philippines to the CSA.  The United States remains in Puerto Rico.  On March 3, 1900, the State of Cuba is admitted to the Confederacy.
