My guess : mainly Southern White counties?

Almost all are in West Virginia, Kentucky, Arkansas and Tennessee.

Lord Admirale:
I immediately think Appalachia and TN/AR. Probably a handful in the rust belt as well.

Stranger in a strange land:
Atlas Favorites Elliott County, KY & McDowell County, WV are surely up there.
