I'm pretty sure that the first option played a role--Trump is a scumbag of the first order. 

But I'm thinking that the personal insults against the Bush children was the primary reason.  His love for his children was legendary and genuine, and this love was reciprocated in turn.  I remember on Election Night 1994 when W won the governor's race in TX and Jeb lost the race in FL.   He and Barbara appeared on TV and were depressed because of Jeb's loss--not really discussing W's win or the big Republican night.     They were more focused on their child's setback--like any good parent would.
