The winner is probably Coleman Andrews winning one county and 2% in Tennessee in 1956.

Alachua County, Florida, in 1928 doesn't qualify, but it does come close:

Smith (Republican) - 35.01%
Thomas (Socialist) - 32.18% (1.59% statewide)
Foster (Communist) - 30.45% (1.46% statewide)

Not Presidential, but look at Lugar's libertarian challenger's results in this election.  He somehow won a county getting about 10% of the vote.

That was a clerical error on CNN's part.  Osborn's vote count in Warren county should have been 341, not 3,419.  It's likely that a data entry guy accidentally brushed the 9 key when going to hit enter and didn't bother to double-check Indiana results, thinking the state inconsequential.
