I am not counting any of the islands that I merely drove across, like Yerbe Buena Island or all of those keys in the Florida Keys between Key West and the Florida mainland. There's no point in counting those islands because I didn't set foot on them or visit them. (There are three islands that I've been to but do not remember their names, I just remember what country they belong to.)
My answer to the poll is 21-30, because I have visited 25 islands (that I can remember).

United States and territories: Santa Catalina, Theodore Roosevelt Island, Key West, Sanibel, Captiva, Hawaii, Oahu, Kauai, Long Island, Manhattan, Staten Island, Wizard Island, Puerto Rico, St. Thomas.

International: Nassau, two other islands in The Bahamas, Mykonos, Rhodes, Crete, Santorini, another Greek island, Tahiti, Moorea, Bora Bora.

I'd say about 35. 25 or so in Europe, 5 in Asia and 5 in the USA and Canada.

I wouldn't know how to count them all. I can say that I've probably set foot on islands in at least half the states and driven across some in many others. I've definitely walked on islands in three Canadian provinces and eight other countries, three of which were island nations. I've even been on islands separate from the main capital island in island nations (most recently spending two nights on Vestmannaeyjar while touring Iceland in 2022).

It's possible that I've been to more, but those that I know of:

Great Britain
Île de la Cité (Paris)
Cozumel (Mexico)
Grand Cayman
Jamaica
Hispaniola
Estero Island (FL)
Long Island
Rhode Island
Prince Edward Island
Island of Montreal
San Juan Island (WA)
Vancouver Isaland (BC)
Revillagigedo Island (AK)
Baranof Island (AK)
Various lake islands

I went to Coronado “Island,” CA once as a kid, and I was disappointed that it was just a peninsula.

Every permanent island I can recall being on (traveling without exiting excluded)

All are in NY, except the last one:
Grand Island
Goat Island
Luna Island

Manhattan
Long Island

Dog Island
DeCamps Island
1 unnamed adjacent island

Heart Island
Casino Island
Another small, evidently unnamed island in the St. Lawrence.
At least one of the islands of the Long Sault Parkway

By massively stretching the definition of "island" you could also count the part of Utica bounded by the Mohawk River, the Erie Canal, and the Utica Canal Terminal Harbor, as well as the part of Ottawa between the Rideau Canal and the Rideau River.

Fun factoid of the day:  I visited Ichabod Crane's gravesite on Staten Island once.  That was kinda cool.

Moments of Zen.

Either than partying literally a stone's throw away from Steven Seagal's empty-ass mansion off Hylan (Strawberry lane, Berry street or WTF ever), that was my highlight of Staten Island.
