For essential healthcare/frontline workers:  the Fed govt buys it, then distributes to states once they have a distribution plan in place  

For most people:  medical facilities/orgs/whoever buy it, the insurance companies (including Medicare and Medicaid) and/or patients pay for it

For vulnerable populations (the VA, Indian reservations, etc.):  the Fed govt buys and distributes it through existing programs for these groups

lol at everyone saying we should use the military.  Our military has better things to do, and it's not like our country is lacking existing healthcare infrastructure to distribute a vaccine.

It would be too demanding and inefficient for the federal government to do everything for everyone outside of existing programs like Medicare or the VA. Healthcare providers should be involved in procurement and distribution to the general population, and insurance companies should pay for it. In the case of uninsured people, the federal government should reimburse healthcare providers, because it's within the immediate national interest to do so.

I can understand using the military, with its organizational infrastructure and manpower, to achieve certain domestic goals, but I don't see how that applies to distributing a vaccine. Maybe using the Civil Air Patrol to deliver vaccines and healthcare professionals to remote communities or something like that, at most.
