What would this unlikely alternate scenario look like? Kavanaugh gets blocked (all Democrats vote no; Republicans Portman, Collins, and Murkowski vote no) and Trump accidentally picks a moderate conservative, who upholds Roe v. Wade. Who would win? Would Trump's base abandon him?

They wouldn’t abandon him, and it wouldn’t be accidental. Trump could get a lot of good press from appointing, say, Jerome Holmes to the Supreme Court. An African-American, supportive of SSM but mostly conservative, is a dream pick for many young conservatives.

Really? Maybe I'm just overwhelmed with cynicism but I am under the impression that they would prefer to see people like Ben Shapiro or Jordan Peterson be appointed.

I think the ideal pick for Trump would be Ted Cruz, actually. Almost every Republican (except maybe Murkowski/Collins and even I think they vote aye) would vote for him in the Senate, and a more "normal" Republican would easily beat O'Rourke.
