Last time I can recall two incumbent House Democrats losing a U.S Senate primary to a third candidate: 1996 when Victor Morales in Texas defeated whichever two U.S House Democrats he defeated.

Jim Chapman was one, forget the other

It shouldn't have,  Mark Harris is underfinanced, but lost by only 134 votes to Pittenger in 2016.

That would be the one place where you'd expect Swearengin to do well though. That's one of the very few places in WV where the Dem base is closer to the national Dem base than the typical WV Dem base.

Well he still has two thirds of the vote so I don't think him slightly under-performing in a single county makes this night a disaster for him.

As I said earlier, places with the largest gap between Manchins 2012 performance vs Obama are giving Manchin the best results in the primary.
