Does this pollster have a good reputation?

Don't know much about them, they seem to be new.

The latest poll I can find from them showed Ted Cruz leading by 10. He won by 14 or so.

Why would Johnson have twice as many unfavorables as favorables - but Obama and Romney both have net favorables? And yet Johnson gets a full 5%?

3rd party candidates usually get higher unfavorables than favorables. Rasmussen shows the same:

http://www.rasmussenreports.com/public_content/politics/elections/election_2012/election_2012_presidential_election/august_2012/16_have_favorable_opinion_of_libertarian_candidate_gary_johnson

But, those who see him favorable, are very committed to him and end up voting for him. Or at least most of them.
