Who qualifies for the berths? I assume that the best team out of each of the 9 groups qualifies. Are the other 5 selected from runner ups with the best results in the groups? In particular how do they compare results from groups with different numbers of countries?

According to wiki all but one of the second place teams (after the round robin) go into another group.  Those 8 teams play each other in a one off make it and your in, lose and you're done.

I'm glad Portugal got an easy group.

Also England vs Scotland is FREAKING AWESOME.
My guesses:
A-Netherlands, France
B-Portugal, Switzerland
C-Germany, Czech Republic
D-Serbia, Austria
E-Romania, Denmark
F-England, Slovakia
G-Italy, Spain
H-Belgium, Greece
I-Croatia, Ukraine
