As a person who will no longer inhabit this piece of dust I call my body, I would say no, however, I would make every attempt to express my wishes to my next of kin and those at my bedside, if I am so fortunate to go peacefully.  I voted "yes" based on the last statement.

Mormons don't believe this actually converts you, just gives you a choice in the afterlife.
