He has/had diabetes and got dialysis for them.  He would need dialysis for treatment, and likely lost a foot and suffered very low blood pressure if he didn't get them.

I said alive in Pakistan, but there's a good chance he was killed or died of diabetes.  Dialysis patients only last a few years anyway, I believe.
