I think a lot of the inconsistency comes from an inconsistency in which posts are reported. I rarely infract things that no one reported; I generally only do so for truly egregious violations.
And, as Kal said, an inconsistency in what different Mods think is acceptable discourse factors in too. My instincts are normally not to modify or infract much of anything that isn't a black-and-white interpretation of the ToS, but I've been trying to apply the rules others have outlined and follow for the sake of consistency.
TJ made an important point here. Some inappropriate posts simply slips under the radar. You may ask "where were the mods?", but the TOS (
you can verify me here) clearly states we're not going through every post.
Personally, I do read most of new posts on my board, but these aren't the busiest one. I pity a mod of ant active election board

And yes, there's no way a group of individuals can have exactly the same outlook. There are some absolutes (no porn, no unfiltered profanity, no copyright violation, no threats), but the rest is, well, open to interpretation. I'd like to ask our critics to put themselves in our shoes for a moment and consider this.
