I would create a capped payroll tax like social security, that is the tax only applies to income up to a certain amount. The income and tax levels would be set to provide sufficient funding for basic health insurance for everyone. The basic package would be something between the current bronze and silver levels.

An exchange would provide everyone access to health insurance from a qualified provider. Everyone could register for basic coverage at no charge. If an individual didn't register they would be assigned one of the qualified insurance providers. The exchange also allows the purchase of additional optional coverage above the basic level, and the options can be bundled or offered as stand alone riders depending on the company making the offer. Employers would no longer play a central role in providing health coverage to employees, though they could offer to pay towards optional coverage or deductibles as a benefit of employment.

When I say everyone would register or be registered, I mean everyone including current Medicaid and Medicare recipients. Everyone is entitled to the same basic coverage. Low-income individuals would have their deductibles covered through Medicaid/Medicare, but otherwise have the same basic plan as everyone else. The reduces the cost of Medicaid and Medicare to just the deductibles and provides some offset for the increased tax to fund the universal basic coverage.

I'm not hearing anyone really addressing the fundamental issues in the healthcare market.

Which issues do you mean? I thought I covered the main market issues in my proposal, though not at the level of detail that you provided in yours. I was trying to avoid a tl;dr, but I'd be happy to address specific details that I glossed over.

Single payer system just like European countries.

Single payer is not used in every European country. As I read your avatar you would know that Germany is not single payer, and like Netherlands have universal multi-payer systems. I think universal coverage is far more important than single payer in a system.

Correct; I meant universal health care like here in Germany.

I wanted to point out that distinction because I have met many on the left who believe that the only way to universal coverage is with a single-payer system.
