My precinct in Bellingham, WA came sort of close:

67% Clinton
13% Dole
11% Nader
3% Perot

I don't have the numbers, but it almost certainly happened in Waldron, San Juan County, Washington. Alcon can confirm this. It probably also happened in the Evergreen State College precinct(s) in Olympia, WA.
