Then why doesn't he called her "Secretary of State" Clinton? That's like calling David Cameron "Prime Cameron".

You can't just omit words willy-nilly.

Comparing apples to oranges. "Secretary of State" can definitely be shortened to "Secretary", as Hillary Clinton was the Secretary of the Department of State.

The point is that secretaries tend to be female jobs. So by referring to Hillary as a secretary, he is implying she should stick to more "woman" roles.

In Obama's current cabinet, male secretaries outnumber female secretaries almost 4 to 1. Where are you trying to go with this?

96% of secretaries are female.

The definition of 'secretary' you're using and the definition of 'secretary' used in the term "Secretary of State" mean two very different things.
