Nintendo Hard: Bleh, it's hard to pick from all the stupid platformers of the 16-bit era.
For actually legitimately challenging, I'll have to say a largely forgotten squad-based strategy game called Tactics Ogre, which was first released only in Japan on the Japanese equivalent of the SNES and got a PSOne port. Think Final Fantasy Tactics except the story is better and makes more sense, and the battles involve much more characters and allow for more strategy. It's really time consuming and a huge commitment, 70 hours is probably the minimum time you'll have to play to beat it.
There was also a horror/adventure game for the PSOne called D that was also pretty hard but due to an annoying set up: You have exactly 2 hours to play it (no saving or even pausing) and if you fail to beat in that time you lose and have to start all over.
