Eventually we will again I believe, but I think it may be as early as 2016 depending on the candidate.

Extremely unlikely, but I'd keep my eye on Paul Ryan if anyone.

1. The Speaker of the House succeeds to the Presidency in the aftermath of a combination of deaths, impeachments and conviction, or resignation.

2. The US Congress goes unicameral (which would require a Constitutional Amendment).

3. Rigged election.

... The House of Representatives is two steps at the least from the Presidency.  Usual preceding steps are either  Vice-President, Senator, Governor, Cabinet Secretary, or highly-successful General  (maybe Admiral).

Representative, State Attorney General, Secretary of State (of a State), and mayor (even if it is Wasilla, Alaska)... are frequent second-to-last steps.
