I see Wilson getting absolutely hammered in this election.  He would have been unable to campaign, and Harding's "Return to Normalcy" argument would have been even more effective against Wilson himself, and Wilson loses 429-102.

It would be a Harding LANDSLIDE. President Wilson was so unpopular, I doubt he would have even got the 1920 Democratic Nomination, if he even wanted it. I can't make a map now but I will make one later.

Wilson wanted to run in 1920. If he didn't have his stroke he would have made a good run for it.

Either way, he loses BIG TIME.

Most of the south in that era was so Democratic, any Democrat would have taken by far most of the south, even Wilson.
