The restaurant should interpret it as the customer leaving $20.13 in tip, and charge him $43.13.
Not 100% sure, but I would guess the credit card companies only care about the total, the total was already written.  If they put in a different total, they are possibly committing credit card fraud.

Seriously, you can't do that.
Yeah, and not even because of the legal reasons, for the moral reasons too.  You're not supposed to steal, even from bad people.  Yes there are scenarios where stealing is morally justified, but this ain't of them.
