For those who haven't seen the movie, the protagonist keeps repeating the same day over and over (until he learns a 'lesson'). The day resets, and any consequences are erased. If he kills someone or blows up a building, they're alive the next 'day' and the building remains intact.

Would you be required to act ethically in such a scenario?

Yes. Because it's about the journey not the destination. The process. Living each moment as God intended, and separate your intentions from any consequences of reward or behavior, but simply because it's right.

I can confirm that if I was in Groundhog Day I would live every day like it was Grand Theft Auto.

This is arguably the most spiritual mainstream/cult movie of the past few decades.

The whole point of the film wasn't who died, or how often he killed himself but how long it took Phil to become a better person.

That he was morally reckless didn't stop the loop. Killing himself didn't stop the loop. It was all just wasted time. However he had to learn that by being reckless. Even when he reached the point where his life was bliss; he knew everyone in the town, learned new hobbies, enjoyed his days and was arguably the most ethical and moral person in the whole town, the loop continued. Two completely different people from start to finish with two different paths but the end result was the same.

Can he permanently die in such a scenario?

Or sustain injuries that do not disappear on the next repeat day?

Did you watch the movie all the way to the end? Within the context of the movie itself, the answer is yes.

Yes. The Good, although most difficult to know, is sovereign, and no valueless libertine pragmatism can supersede it.

The problem would be knowing when the loop was going to end.

Yes, because moral choices mark the soul, with good choices healing it and evil ones damaging it. If you remember past loops, the marks would remain along with your memories.

Should I watch this movie? I suspect I might find it irritating.

Yes, the movie doesn't approach the moral implications all that much. It's a rom-com and focused on self-improvement.
